Test and Exam Accommodations
There are a variety of accommodations that the Fred Smithers Centre can provide clients. These may be extra time on tests and exams, or assistive technology. The accommodations that a student receives is based on their documentation, and an individual plan made at the intake meeting with an Accessibility Advisor.
Accommodations may be necessary to ensure that students are able to reach their academic goals. We want to ensure that all students have equal opportunity to do their best by providing appropriate accommodations. These accommodations are based on the documentation of disability provided by their Doctor or Psychologist that was provided at intake and may be updated during their university career.
Quizzes, Tests, and Midterms
It is the student’s responsibility to request test and exam accommodations. A separate request form must be filled out for each test/exam.
The Fred Smithers Centre requires at least two week’s notice for quizzes, tests, and midterms. Two week’s notice means that requests must be submitted 14 days before the test date. Due to the volume of requests that we receive, late test requests may not be accepted and accommodations may not be granted.
The test/exam request form can be completed here. Once completed, the Fred Smithers Centre will be in contact with your Professor.
Final Exams
Students need to make arrangements for final exam accommodations through the Fred Smithers Centre once the final exam schedule is posted by the Registrar's office. This can be found on banner under concise student schedule.
Final exam requests for Fall semester must be submitted no later than November 18th.
Final exams for Winter semester must be submitted no later than March 17th.

Online Exams
If you are taking a course with online tests or exams make sure you contact the Accessibility Advisor at the beginning of the semester to notify your Professor of your required accommodations. The Test Request Form is not required for an online test unless it needs to be proctored.
