Not all browsers are compatible with fillable PDF forms. Depending on your browser you are using, you may see different results when completing a fillable PDF because of the built-in PDF viewer. Adobe Reader is the only program that will allow you to work with this form properly and enable the REB to access the information. Mac and Chrome users should not use Preview. Save this form (PDF format) on your computer. Right-click the PDF and select Save as or Save page As. After you have completed the form, save the final version of the file to your computer.

Need help with determining whether a plan needs to be submitted for REB review?
The TCPS 2 (2022) suggests that when in doubt about the applicability of the Policy to a particular project, the researcher shall seek the opinion of their home REB. Feel free to work together and check with us anytime even when only part of a plan has been designed. We're here to help and advise at early stages. This type of request is optional, however should be the Step 1 interaction with the REB to learn whether the plan is considered "research" with "human participants" according to the TCPS 2; that therefore requires REB clearance (the submission of a Form 1 or Form 1 C application), or is exempt from REB clearance (not requiring the submission of a Form 1 or Form 1 C application.) A "review" of the intentions is provided for a request, regardless of the two possible outcomes of this type of review. Applicants are strongly advised to obtain this evaluation and receive a formal evaluation from the REB. The REB response to these type of request is quite quick (a few days). A fresh request must be submitted when changes to the (previous) intentions arise, the REB will reassess and consider the ("current") new plan.
